Abstract Crohn's disease (CD) is a chronic, relapsing, idiopathic condition, characterized by granulomatous, transmural inflammation of the gastrointestinal tract, which can affect its entire length, from mouth to anus. Metastatic (MCD) rare form skin involvement and defined lesions without contiguity with tract. A 9-year-old patient presented complaints gross in vulva perianal region. The diagnosis was made when 11 years old, after being evaluated colorectal surgeon. Treatment started “top-down” approach, sustained response for four years. Afterwards, there relapse previously normal areas, overt symptoms. consisted steroids local infiltration infliximab, improvement. year later, rapid progression lesions, drug changed adalimumab, also worsening lesions. admitted hospital intravenous were initiated, along surgical debridement After some improvement, ustekinumab initiated satisfactory response. Pediatric MCD has an important impact on patient's quality life, influences growth social development.
